Assumption of Risk

I understand that the Beaver Institute’s BeaverCorps Professional Training Program, in which I have chosen to participate, involves certain risks, including but not limited to legal liabilities, property damage, and bodily injury to self or others. With acknowledgment of the potential risks involved with this activity, I expressly and knowingly release, hold harmless, and agree to indemnify Beaver Institute, Inc. its employees, representatives, officers, advisors, and agents, from any and all claims and causes of action for property damage, personal injury or death sustained by me and/or my associates and/or caused by me and/or my associates arising out of this activity.

I also understand that I am responsible for adhering to any and all federal, state, municipal, and environmental rules and regulations connected to this activity, along with the instructions provided for this activity that are contained in the training provided by the Beaver Institute, Inc. (BI). In addition, I understand and agree that BI cannot be expected to identify and control all risks associated with this activity.  I further acknowledge that I possess my own personal insurance to cover any accidents or injuries associated with this activity.

Payment Agreement

I understand and agree that I am financially responsible for the payment of the BeaverCorps Professional Training tuition fees established by the Beaver Institute. I agree to pay the Beaver Institute in the time period established in my payment plan.
